INSTRUCTIONS: Visitors and members of the congregation may sign up for reserved seating. Worship Service Personnel register using the designated registration option on the next screen Welcome to our church! We look forward to seeing you in service!. Mt. Pisgah is committed to providing a safe environment for all who enter. By registering for this event, I agree to answer all questions in the registration form and in addition I hereby attest to the following: I understand that I am asked to electronically pre-register for Worship Services and building entry. I understand that if there is no pre-registration, one will be required to complete a manual questionnaire on-site. I understand upon arrival, the process is to ensure the mask is on before entering the building, sign the Trustee – Guest book, sanitize hands, step before the thermometer for the temperature, and receive the green go light to proceed to enter the sanctuary. I understand an N95/K95 mask is required for all entering the church building. Masks will be provided to anyone that is in need. I understand that I am to follow all the directions of the ushers in seating and movement in and out of the sanctuary. I understand that there will be no congregating in the halls of the church building; and will follow all protocols for restroom access. I understand that at the completion of service all worshippers are expected to follow the directions of the Ushers to exit the building quickly. I understand that attendance at this Worship Service is at my own risk and discretion and I will hold Mount Pisgah Harmless.
