Columbia/Boone County Public Health & Human Services is pleased to invite you to attend the kickoff of the Live Well Boone County Community Health Partnership. This partnership works together to create a community health assessment and improvement plan that has been used by agencies to leverage dollars to enhance services and programs. Since its inception in 2013, Live Well Boone County has been a partnership of public, private, voluntary, and non-profit entities that have contributed to providing public health services across Boone County. Members of this group have provided input to identify community health priorities and to ensure their constituents’ voices were reflected in the process. The meeting will be held on Thursday, November 17, 2022 from 2pm-4pm at Memorial Baptist Church located at 1634 Paris Road in Columbia.
