I need a slab poured for a shop foundation

I need a slab poured. 13 feet by 30 feet. The footing will be at least 14 inches deep and rebar reinforced. The slab will be 4 inches deep...no rebar required. It will be 3 inches above grade and a monolithic pour. It will be an early morning pour. There is a large alley behind the house that gives perfect access to the work area. The job site is in the area of 24th St and Indian School. Please send me an estimate based solely on the information provided. I work full time so unfortunately I do not have time to have contractors over to the house for an on-site estimate until after I receive the first quote. Yes I do understand that the numbers can change dramatically. I prefer licensed but that's not a deal breaker...examples of your work always helps.
