Acknowledgment of Risk I hereby acknowledge and agree that myself, my spouse, and my child’s participation in GPCC activities comes with inherent risks. I have full knowledge and understanding of the inherent risks associated with GPCC participation, including but in no way limited to: (1) slips, trips, and falls, (2) aquatic injuries, (3) athletic injuries, and (4) illness, including exposure to and infection with viruses or bacteria (including COVID-19). I further acknowledge that the preceding list is not inclusive of all possible risks associated with participation and that said list in no way limits the operation of this Agreement. Photo – Video Release The participant is allowed to participate in any way in this program, related events and activities: therefore, I agree that participant’s likeness may be photographed or videotaped and that such image may be published in an outlet used to promote or publicize the program/activity. Waiver, Release, Indemnification & Covenant Not to Sue In consideration of myself, my spouse, and my child’s participation in GPCC programs, myself, my spouse, and my child, the undersigned participant, agree to release and on behalf of myself, my heirs, representatives, executors, administrators, and assigns, HEREBY DO RELEASE GPCC, its officers, directors, employees, volunteers, agents, representatives and insurers (“Releasees”) from any causes of action, claims, or demands of any nature whatsoever including, but in no way limited to, claims of negligence, which myself, my spouse, and my child, my heirs, representatives, executors, administrators and assigns may have, now or in the future, against GPCC on account of personal injury, property damage, death or accident of any kind, arising out of or in any way related to the use of GPCC facilities/equipment or participation in GPCC programs whether that participation is supervised or unsupervised, however the injury or damage occurs, including, but not limited to the negligence of Releasees. In consideration of myself, my spouse, and my child’s participation in GPCC, myself, my spouse, and my child, the undersigned participant, agree to INDEMNIFY AND HOLD HARMLESS Releases from any and all causes of action, claims, demands, losses, or costs of any nature whatsoever arising out of or in any way related to my GPCC participation. I hereby certify that myself, my spouse, and my child have full knowledge of the nature and extent of the risks inherent in GPCC participation and that myself, my spouse and my child are voluntarily assuming said risks. I understand that myself, my spouse and my child will be solely responsible for any loss or damage, including personal injury, property damage, or death, myself, my spouse, and my child sustain while participating in GPCC and that by signing this agreement myself, my spouse, and my child I HEREBY RELEASE Releasees from all liability for such loss, damage, or death. I further certify that myself, my spouse and my child are in good health and that I have no conditions or impairments which would preclude my safe participation in GPCC. I further certify that I am of lawful age and otherwise legally competent to sign this agreement and that I have legal capacity to act as the parent/guardian of my child. I further understand that the terms of this agreement are legally binding and certify that I am signing this agreement, after having carefully read it, of my own free will. Park Rules 1. All GPCC buildings are smoke free. This includes electronic vaping devices. 2. All persons entering GPCC do so at their own risk. 3. Children under the age of 13 must be accompanied by an adult. 4. Entrance to GPCC will be refused to any person under the influence of drugs or alcohol. 5. Gang attire, firearms, air guns, explosives, fireworks, or knives are not allowed within the park. 6. Bicycles and motorcycles are allowed inside the park, but must be parked in designated areas 7. Posted speed limits and other traffic laws must be observed. 8. Obscene and profane language will not be tolerated. 9. Utilize all equipment and facilities according to their intended use. 10. Comply with posted signage and verbal directions of GPCC staff. 11. Bathing suits are only allowed at the GPCC Waterfront and bathhouse. 12. Food and drinks are allowed in designated areas only. 13. Litter must be disposed of in trash cans. 14. Defacing property or vandalism will result in forfeiture of GPCC privileges and appropriate legal action will be taken. 15. Collection of money or sale of products on the premises will not be allowed. 16. Any person not conducting themselves in an appropriate manner will be required to leave the premises. 17. Rubber and steal cleated shoes are not allowed in the buildings. 18. Tennis shoes must be worn in the gym and racquetball courts. 19. All persons must be dress in appropriate and suitable attire. I certify that all fees have been paid. I further certify that I have read, understand, and agree to comply with terms and conditions of this waiver. I further certify that I understand and agree that a violation of any terms and conditions of this waiver may result in termination. It may also cause a suspension of all facility privileges. The purchase of your ticket is your signature to the above waiver.
