We need someone to go to the Health Department, which is located at: Guernsey County Health Department 326 Highland Avenue Cambridge, Ohio 43725 We will provide you with the application and the fee to pay for it and you will send it back to us with certified overnight shipping. We will provide you with the mailing label so you just have to drop it off at the Post Office. We are paying $50 for this task. If this post is still active then the task is still available.
