Please submit this form for review. Residents need to submit a $25 payment whether by check, money order (in person only to the HOA office made payable to Teravista HOA) or through this eventbrite. Booth must be approved before 11/9/22 in order to participate. Please note that you would need to provide your own equipment ( tent, table, chairs, etc. )
