Have a valid U.S. driver's license and/ or reliable transportation. Have the full support of her spouse/partner. Not use drugs, smoke cigarettes, or abuse alcohol (the same applies to anyone in your household). Be willing to have IUD removed. Women with IUDs/the shot are welcome to apply, and should understand that they may be placed on hold until medical clearance is given.
