Join us at Lake Las Vegas Sports Club on Saturday October 22nd for our 4th Annual Pinktober festivities to help raise awareness and funds for the fight against cancer. By the execution of this agreement, I accept and assume full responsibility for any and all injuries, damages (both economic and non-economic), and losses of any type, which may occur to me or my guest, and I heby fully and forever release and discharge the company, its insurers, employees, officers, directors, and associates, from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out the use of said equipment and facilities. I expressly agree to indemnify and hold the company harmless against any and all claims, demands, damages,rights of action, or causes of action, of any person or entity, that may arise from injuries or damages sustained by me or my guest, including sickness or other related injuries potentially caused by COVID-19. I agree to be solely responsible for the safety and well being of my guest and myself. I understand that the company does not provide supervision, instruction, or assistance for the use of the facilities and equipment, and that the use of exercise equipment involves risk of serious injury, including permanent disability and death. I agree to comply with all rules imposed by the company. In addition, I agree to follow the current guidelines issued by the state and CDC in regards to COVID-19 related concerns. Please read the following and sign when registering for this event: *I do not knowingly have the coronavirus, have not knowingly been exposed (within 6 feet for 15 minutes or more) to anyone that currently has the virus, and do not currently show symptoms such as a dry cough, shortness of breath, repeated shaking/chills, muscle pain, sore throat, vomiting, diarrhea, loss of taste/smell, headache, fever, etc. I HAVE READ THE FOREGOING WAIVER AND RELEASE OF LIABILITY AND VOLUNTARILY EXECUTED THIS DOCUMENT WITH FULL KNOWLEDGE OF ITS CONTENT.
