Our Annual Flu Shot Clinic is back. Flu Shots are available to all employees and eligible dependents who are enrolled in any of our group medical plans (Anthem Blue Cross PPO, Anthem Blue Cross HMO, Anthem Bronze, Anthem Minimum Value PPO & Kaiser HMO). You MUST register using the link below to print your ticket. You will receive an immunization consent form, via email, a day before the event that must be completed and printed before you arrive to the Flu Shot Clinic. The clinic date will be on Tuesday, September 27th from 10:00 a.m. to 1:00 p.m. in the Creveling Lounge (denoted as location “CC” on the campus map https://pasadena.edu/about/campus-maps.php). You may walk-in at any time between the times provided. In order to maintain a safe environment, please make sure that you complete the Daily COVID-19 Screening for Employees and that you are approved to be on campus, before attending the clinic. If an eligible dependent will be attending the clinic, they will need to scan the QR codes displayed throughout our campus to complete the survey prior to their arrival to the Creveling Lounge. You may be asked to show your clearance before entering the Creveling Lounge. Also, please remember to social distance the day of the event.
