Advice needed

Not homeless, on SSI disability and not in best shape, have a question, does anyone know of any places that help with food, and provide transportation to and from doctor's appointments? My whole check is spent, and really need to see doctors? This would be for Henderson and Las Vegas. Thanks in advance!!
