This is a COVID-19 Test Site Sign up provided by Allied Health. Testing is for anyone ages 5 years and older. Individuals under age 18 must have a parent or guardian present.Please bring your ticket with you in order to receive testing. Each person receiving testing is required to sign up for an appointment. Each person must bring a form of identification (driver's license, SSN, passport etc.). You must bring your insurance card if you have insurance. If you are uninsured, you will be asked to sign a document verifying you do not have insurance prior to receiving a test. NOTE: Please do NOT sign up for this event if you need a result for medical procedure. Lab result times cannot be guaranteed before a procedure. Consult your physician for the best testing option.
