**DO NOT COMPLETE THIS FORM IF YOU ARE WANTING YOUR FIRST DOSE OF COVID VACCINE, YOU MAY WALK IN BETWEEN THE HOURS OF 9AM-3PM ON 9/15/22** Moderna Bivalent Booster doses are available to those 18 years and older who have completed a primary series or a booster dose at least 2 months ago. If you recently were diagnosed with Covid, please consider waiting 3 months to receive the booster due to limited quantities of the vaccine. After receiving the vaccine you will be expected to be observed for 15-30 minutes in our office.
