COVID-19 Vaccination Drive Thru for Priority Groups (65+ and health care workers)
Vaccination is for anyone ages 65 and older and health care workers with direct patient contact.
Anyone under age 65 will not be vaccinated without proof of health care worker status.
Appointments are required for this vaccination clinic and will not be scheduled on site. Do not arrive at the vaccination site without an appointment.
Each person receiving the vaccine must sign up for an individual appointment ticket. Bring the ticket with you to receive your vaccination.
Each person must bring a form of identification (Driver's license, passport, etc.)
Each person receiving the vaccine must complete a vaccine screening and consent form to confirm eligibility. These are available on our website. For those that cannot access the forms in advance, a paper copy will be provided at your appointment time.

FIRST MODERNA DOSE JANUARY 11, 2021, SECOND MODERNA DOSE FEBRUARY 8, 2021
Second dose occurs at same location and ticket time on February 8, 2021
This is a drive thru only, please follow prompts on-site and do not exit your vehicle unless asked to do so by staff.
