Oakland Community Health Network (OCHN) is requesting bids from a single contractor qualified to provide HIPAA-complaint document shredding services including all labor, materials, incidentals and means of work as described in Attachments A, B, C, D, and E of this Request for Proposal (RFP) OCHN invites Contractors to submit proposals in accordance with the terms and conditions of this Request for Proposal (RFP). The RFP provides the requirements and evaluative criteria for services and requests a detailed response from all prospective Contractors, including pricing and service descriptions in a specified format. To be considered, a Confirmation of Interest Statement shall be sent electronically by 5:00 p.m. on November 19, 2019 to Brittany Campbell, Administrative and Special Projects Coordinator at [email protected].
