Small personal loan - Have collateral

Recently came into some hard times with an unexpected ambulance transport bill with no insurance.. I need a small $4000 loan for one month with a interest of $500 at the end of the loan for allowing me to loan. I work as a general manager at a local restaurant and have paystubs, current, to prove I can pay back. I've never done something like this so i'm not even sure what to expect but I appreciate any help! I have some collateral as well!
