As a parent I am aware that is hard to leave your child(ren) with anyone. Times have changed and it is hard to find responsible help. I have over 25 years of experience working in childcare. I am a former employee of the Charlotte Mecklenburg School System. I have also been a licensed healthcare professional for over 10 years; I am a trained and experienced first responder. *I require full documentation and registration for each child. Absolutely no exceptions. ~~~~~What I will need from you~~~~~ *I will provide these forms 1. Child(ren's) Information 2. Parents contact information a) please inform me of any legal custody agreements as necessary. 3. Emergency contacts information 4. Emergency contact consent form a) current DL or ID and name on consent form must be present for the pickup of your child(ren). b) we will be releasing your child(ren) to the person's listed by you, because a significant circumstance has occurred with the caregiver, parent, child or at the caregivers location. 5. Medical history/emergency agreement 6. Allergy/ Food restrictions 7. Scheduling agreement 8. Deposit *If you are not able to provide this documentation I will not be able to offer services to you at this time. Note to parents: I do not offer emergency (last-minute) childcare, unless the child(ren) has already been registered with me, the deposit has been paid, and we have already had our meet and greet. *I have 2 full-time 3rd shift slots available for childcare. *2 Monthly Date nights as well.
