We are doing a Bread Taste Test Study on Friday, August 26th.  It is 30 minutes and you would be paid $60.00 in cash.  This study will take place downtown Salt Lake City. What you would be doing is tasting bread with Turkey and cheese and answering questions on a computer.  If you would like to participate.  Please answer the questions below: Questions: Name: Age: Email Address: Phone Number: 1. If you qualify to participate in a 1-day market research survey, you will be asked to taste several bread products and give your opinions. The interview will take 30 minutes would that work for you?  8. During this visit we will ask you to eat several products so please take that into consideration when planning what you might eat or drink before coming to the interview.  Would this work for you? 9.To ensure the health safety of you and others, we have a few questions to ask you. Have you….-       Been experiencing any illness, fever or feeling unwell?-       Been infected with COVID-19 within the past 14 days and are recovering?-       Had unprotected exposure to someone with COVID-19 or to someone who has been asked to quarantine for possible exposure during the past 14 days? 10. Do you currently have any medical condition that restricts your use of some foods and beverage products or affects your ability to evaluate them? These conditions include, but are not limited to cold or nasal congestion, diabetes, food allergies and sensitivities (such as gluten, dairy, eggs, fish, shellfish, tree nuts, peanuts, wheat, whole grains, flax, seeds and soy), allergies to artificial sweeteners of any kind, caffeine allergy, sulfite allergy, (Female only) pregnancy/lactation, or any other medical conditions? 11. How comfortable are you working with a computer/ laptop and mouse?  12. Since you will be doing some reading, if you wear glasses, please bring them with you.  Does  this present a problem?  13.
