Join us for our first PCHS Pink Out the Pace Color Run! This event includes a 5K run/walk & 1 mile kids fun run. Check-in begins at 8 am. The 1 mile kids fun run will start at 9 am with the 5K run/walk following. Proceeds from this event will go to our newly created PCHS Cancer Fund. Early detection of cancer increases a person's survival rate. Due to this, we want to make sure everyone has access to the appropriate diagnostic tests. Through the Cancer Fund, Perry County Health System provides various cancer screenings and treatments to residents in our community who may otherwise be unable to afford these potential life saving measures. ASSUMPTION OF RISK AND RELEASE OF ALL CLAIMS By submitting this form I allow my child and myself to participate in Perry County Health System’s Pink Out the Pace Color Run, including transportation provided. I release Perry County Health System (PCHS), and their employees, of any liability, claims or demands, which we may have hereafter as a result of my child's and my own participation and use of recreation facilities. I understand PCHS has no medical insurance for my child or me. I understand there are risks involved with physical exertion, including serious injury and illness (ex: communicable diseases such as MRSA, influenza, COVID-19, head lice, etc.) from the activities involved in this color run and can be significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist. I certify that my child's and my own physical condition are satisfactory to participate in this color run. I give my consent to PCHS to take photos/video of my child and myself to be used by PCHS for color run promotion. I am at least 18 years of age and verify that all the information provided at registration or on this form is correct. Providing incorrect information including, but not limited to, date of birth and address is grounds for removal from the run and may result in suspension of the privilege to participate in future programs.
