Description

Covid Pre-screening Questionaire ALL attendees (Pastors, ministers, Dream Team, guests, etc.) MUST answer questionnaires, obtain a negative screening, negative temperature screening and obtain hand sanitizer and wear a mask over both nose and mouth each visit to campus before entering the facility. Dream team pre-registering would be most helpful. Positive/Symptomatic Attendees Any individual that has a positive screening questionnaire and/or a positive temperature screening (100.5 or above) will be asked to quarantine for two weeks before returning to campus and/or obtain Covid 19 testing and will be provided a Testing Site Handout. Any individual with a positive screening will be asked to provide proof of a negative Covid 19 test prior to reentry before the two week quarantine period is expired. Any individual who refuses to properly wear a mask over both nose and mouth or maintain social distancing will be asked to dismiss themselves in order to maintain safety for all attendees.
