REPAIR: Elevated cement patio leak causing wood facia to crack

The house has an upper elevated patio and a lower patio (still a bit elevated). There is a crack in the cement in the upper patio (we believe) that is causing water to accumulate and damage a wooden decorative panel making it buckle. Your job would be to fix the cement cracks in the patio and fix the wood decorative panel and paint. See pictures. In order to be considered, you must have a website that shows similar types of repair work. Businesses strongly preferred.
