Concrete Chimney Slab Replacement

The concrete chimney cap / slab on our chimney has some cracks as shown in the photos. The job we need done would involve removing the existing Concrete Chimney Slab and pouring a new concrete Chimney Slab, shaped like the old one. The dimensions of the concrete needed to be replaced is 40" X 28". The thickness is 3/4 inch around the chimney edges and tapers up to about 3 inches around the middle. If you have experience doing concrete / masonry work and can do this job, please respond with a bid and photos / details of concrete work you've done.
