Because of the witness requirements, everyone I know is disqualified from signing. I need two people willing to be witnesses on a form. I have kidney disease. I need an Advanced Directive signed, but Colorado law is making it impossible for me to get the document signed because the requirements rule out anyone who is involved with my care. Everyone I know in life.
STATEMENT OF WITNESSES
Neither of the witnesses is my attending physician or any other physician, an employee of the attending physician or health care facility in which I am a patient, a person who has a claim against any portion of the estate of... at his/her death at the time the declaration is signed, or a person who knows or believes that he/she is entitled to any portion of the estate of... upon his/her death either as a beneficiary of a will in existence at the time the declaration is signed or as an heir at law. If... is a patient or resident of a health care facility, neither witness is a patient of that facility. Neither witness is a person appointed as Agent or Alternate Agent by this document. Neither witness is a provider of health or residential care or an employee of a provider thereof.
This requirement rules out everyone I know and I cannot get the document notarized. I need two people willing to volunteer to help me get this document signed. We would meet at my bank, First Bank at E Iliff Ave & S Havana St, Aurora CO on a Saturday.
