Hello, we are conducting a focus group via Zoom Thursday July 28, 2022, from 6:00 P.M. to 8:00 P.M. (Latest). If you are over 18 and a Pierce County resident, we would love to have you participate! Participants will be paid $75 for their time/work via Venmo, Cashapp, Zelle or Paypal. If you are interested, please fill out the following confidential questionnaire and return it to us via reply email. We will make selections and let everyone know by Wednesday (27th) around 2-3 P.M. PLEASE READ: Because this focus group is intended to replicate the demographic of a specific zip code and/or area we will be requiring proof of residence for this group. We are asking all applicants to provide a photo of some valid mail or ID WITH application to be considered. Do you own your own home? 16. Have you ever filed a Labor & Industries claim for an on-the-job injury? If not, what are your thoughts or feelings about the Labor & Industries system in place for injured workers? 17. Do you follow the news? What kind of sources to you tend to favor? Which sources do you think are the most trustworthy? 18. Have you ever been called to serve on a jury in the past or are you currently on call? 19. Do you currently or have you (in the past 10 years) worked in any of the construction trades? 20. What are your thoughts about how brain injuries might affect a person’s ability to work or to think? 21. Have you ever had a physical injury lasting more than several months? (Ex. Back, neck, leg etc.) Final Questions: The following demographic questions are not required but will help us to ensure we have a representative mix of participants in this research. Skipping these questions may affect our ability to determine your eligibility for this research and result in de-selection for participation.
