VIAL OF LIFEThe Vial of Life is a program that allows individuals to have their complete medical information ready in their home for emergency personnel to reference during an emergency. This program is used to provide the patient’s medical information when a patient is not able to speak or remember this information. Vial of Life programs are commonly used by Senior citizens and are promoted by senior center organizations, and fire departments, and other community organizations. The decals, medical information forms, magnetic refrigerator document holder, your photo taken and other materials are free at our office conference room.We will explain the program and provide assistance each week in completing the forms and taking your photo for your free Vial of Live Magnetic Document Holder. The entire program if free and is another example of our commitment to our community and our clients
