Sr. Cat advice or home

Hi there, I am not sure where to go with this, but I’m unfortunately having to move cross country and I have an 12 year old cat that is unable to make the trip at all and I can’t take him with me due to family allergies where I’m staying. Since Covid I have lost my job and have not been able to take him to a vet of any kind and he has an umbilical hernia that needs taken care of as well as the fact that he has lost weight and is biting his fur off. I am not sure but I think he is sick. He acts fine and is the most loving little man around. I call him an attention-hound because he just loves love. I am not sure what to do with him. I don’t want to give him up but I have no choice. Does anyone have advice as to where I could take him to get him help and a home? He won’t be alive much longer and is so loving. We have a dog as well and he gets along great with kids, animals etc. Don’t come at me yelling or harshing at me. I don’t need it and mentally I’m already messed up about this.
