Clinic to be held at the JCC on Wednesday, July 13. Appointments will be scheduled 3:00-5:45 PM. Walk-ins may enter the line 6:00-6:30 PM. Second Dose Clinic will be held Wednesday, August 24, 3:00-6:30 PM (same time as previous appt.) Third Dose Clinic will be held Wednesday, October 19, 3:00-6:30 PM (same time as previous appt.) This is a first dose vaccination clinic for Wisconsin residents ages 6 months - 5 years. Children 6-35 months must bring a prescription from a physician. All minors must attend with a parent or guardian and present a valid birth certificate.
