The Perry Township S.H.A.R.E. Event helps families that need some extra support in preparing for the upcoming school year. Join us again this year to get supplies your children need to start the 2022-2023 school year off with a bang! We will be giving away backpacks and basic school supplies. There will also be vendors and tables onsite to help families with school-required vaccinations, health insurance, free/reduced lunch applications, general health screenings and more. Only one registration per family but you MUST select enough tickets for every school-aged child and fill out the information for each ticket. We will have nurses from Franciscan Alliance onsite for students to get caught up on their immunizations if they are behind, and if you want your student to get their immunizations now in preparation for the following school year (PreK, 5th and 11th graders). This is an optional service. Students who participate in the clinic will be vaccinated with all recommended vaccinations that are due at the time of the clinic. A Perry Township nurse will assist the staff from Franciscan Alliance to look up your student's immunization history so they know what immunizations your student needs. Vaccines that may be administered include DTaP/Tdap, Hepatitis A, Hepatitis B, HiB, Polio, Rotavirus, Influenza, MMR, Human Papilloma Virus, Meningitis and Varicella. The Franciscan Alliance accepts all insurance (commercial, Medicaid and Medicare), and can also provide immunizations for those without insurance at no cost. If you would like to have your student participate in the immunization clinic at the S.H.A.R.E. event, you will need to complete this form. Forms will be available at the event and must be filled out by the parent/guardian.
