What would cause an electrical socket and wiring to extend from the w

Alright I know that it seems pretty unsafe and I am going to deal with that I am just looking for an explanation of how that can happen. Imagine if you will the faceplate, socket, and wire are still connected together and it looks like it was just ripped out of the wall in one piece. I know I didn't do it and I know the person in that room can't do it. I also I have difficult time pushing it back in and I'd rather not fight with it for fear of damaging the wires.
