Grinding Down Concrete

I'm looking for someone who can grind down concrete. This is a sidewalk in front of my house that is misaligned and causing a trip hazard (according to my HOA). I'm pretty handy and have done a lot of hiring on craigslist, but I've never fixed something like this before. Photo attached.
