DOH-Collier COVID-19 Vaccination for Healthcare Personnel with Direct Patient Contact & Persons 65+ Vaccination is for anyone ages 65 years and older and healthcare personnel with direct patient contact. Individuals under age 65 will not be vaccinated. Bring your ticket with you in order to receive vaccination. Each person must sign up for their own individual appointment to receive a vaccine. Each person must bring a form of identification (driver's license, SSN, passport etc.). Healthcare personnel will be asked to provide identification indicating they are healthcare staff.
