AFHS Boys Soccer Annual Fundraiser Futsal Tournament Registration is now open for our annual Midnight Madness @ the Cave futsal tournament. It will be held on Saturday, January 30th, 2021 from NOON to MIDNIGHT at the American Fork High School. Registration closes on Thursday, January 28th. No same day registration will be allowed. Please, register your teams today! Please, pay your registration fee of $100 here. ONLY ONE PERSON PER TEAM NEEDS TO REGISTER -THE TEAM CAPTAIN. By registering you agree to the terms of the waiver found below. Game schedule will be e-mailed to the team captains on Friday, January 29th. About the event: 5v5 teams (including goalie)Guaranteed 3 gamesPlayoffs for each bracketPool play10 min halvesAll High School Teams (9th-12th grade) can be coed. There can't be more than 2 returning AFHS soccer players per team. 5v5 Futsal Tournament Brackets: 5th-6th grade youth7th-8th grade youth boys7th-8th grade youth girls9th-12th grade casual coed9th-12th grade competitive coedAlumniCovid-19 protocols need to be followed. Please, wear a mask when not playing and distance 6 ft. apart. Teams will need to leave the building in between games. We will try our best to make a schedule that is convenient for all teams. Pre-packaged concessions will be sold. Please, join us for a day of fun and soccer! All proceeds go to AFHS Boys Soccer Program. Thank you for your support! -AFHS Boys Soccer WAIVER: AFHS Midnight Madness @ The Cave 5v5 Futsal TournamentWAIVER: I as a participant or parent/guardian of the participant I hereby assume the entire risk and hazards incidental to the conduct of the activity of futsal as well as the transportation to and from the activity. I release, absolve, and indemnify AFHS soccer and AFHS, employees, volunteers, and/or sponsors from all risks and hazards associated with the activity in the event of an injury. I do expressly waive all claims against them. I understand that the AFHS soccer program and AFHS provide no individual insurance coverage. As a parent in this program, I recognize and acknowledge there are certain risks of physical injury and agree to assume the full risk of any injuries, damages, or loss which my child may sustain, as a result of participation in such program as against American Fork High School, Alpine School District, their officer’s agents, servants, and employees. I do hereby release and discharge all the above-mentioned entities from any and all claims from injuries, damages, or loss which may occur to my child from participation in this program/activity. I further indemnify, hold harmless, and agree to defend the above-mentioned entities and their officer’s agents, servants, and employees from any and all claims resulting from injury, damages, and losses sustained to my child arising out of, connected with, or in any way associated with activities of this program/activity. This waiver is also an official release for the AFHS Midnight Madness @ The Cave 5v5 Futsal Tournament to take and display pictures of participants and of this event as they deem necessary to further market the event. All pictures may be displayed on the website and other marketing tools or platforms.
