Job description Summary: The Team Leader is responsible to assure the needs, wants and desires of the individuals supported are being met. The Team Leader is responsible to assure that their respective team works cohesively while at the same time embodies the vision and mission statement, core values and code of ethics of Communitas. Certificates and Licenses: All Training is provided and paid for by Communitas. Responsibilities: Welcoming new Team Members/Mentoring new employees. Participate in interviews for those applying for permanent positions. Evaluation of team members and accountability for progress. Be acquainted with household activities, shift responsibilities and schedules of the individuals being supported. Participate in and model to team members the integration of residents in the community. Relationship development/positive contact with families, guardians, employers and natural supports. Facilitate effective team communication. Facilitate and monitor attendance of staff meetings. Facilitate division of responsibilities. Understanding and utilizing the scheduling tool to meet the needs of the program. Participation in all phases of Person Centered Planning. Reviewing progress notes & monthly summaries for timeliness and accuracy. Review imprest tracking sheets and ensure medication audits are occuring daily. Ensure the team is following Communitas Incident Reporting procedures including guardian notification. Reviewing timesheets for accuracy and approving them. Budget development and upkeep. Utilizing written verbal warning for the purpose of Team problem solving. Review On-site certifications monthly to ensure they are up to date. Ensure medication procedures are understood and followed. Review Medication Administration Record and all medication packets for accuracy. Participate in the development and revision of and understand each resident’s Positive Behavior Support Plan. Coordinate gathering data for revisions to Positive Behavior Support Plans and recommend necessary revisions. Assure staff is prepared for routine medical appointments and there is proper documentation of the appointment. Modeling interactions with individuals supported, assuring empowerment of the individual and flexibility in actions to their needs desires and wants.
