Position Title: Housing Focused Shelter Case Manager Supervisor: Client Services Manager Status: Full-time Description of Position: Boulder Shelter for the Homeless is seeking a self-motivated team player to join our Housing Focused Case Management team. This role will assist individuals to identify and prepare for potential housing opportunities that are safe and sustainable, while utilizing internal and external resources and programs. The position will have an emphasis on developing and promoting resources available to support the goal of finding and maintaining supportive housing. Primary Responsibilities: Housing Focused Shelter-Client Support: • Serve as contact and go-between for clients and outside service providers who are working with the client on housing or other supportive services • Assist clients in obtaining all required documentation for housing while tracking and recording relevant information • Complete and/or update the VI-SPDAT with clients • Assist client in identifying and obtaining public assistance • Make appropriate referrals to community programs and agencies for ongoing supports for client’s mental and physical well-being • Help with requests from current and former clients as well as clients who are referred to BSH by CE but do not use BSH Shelter • Assist with obtaining basic needs as appropriate • Assist clients in applying for all open housing wait lists • Perform weekly Reserved Bed program intakes • Maintain monthly statistics for in-house and external entities • Oversee mail that is received from corrections facilities and coordinate support for those in need • Other duties as assigned Resource Responsibilities: • Maintain and update the Boulder Shelter for the Homeless Community Resource Guide, published monthly or to bring up-to-date when there are significant changes • Distribute electronically to more than 100 individuals who have specifically requested to be on the email list • Maintain communication with representatives from the organizations listed in the guide, as well as Shelter staff • Continually source information to enhance the Shelter’s resources and case management capabilities • Maintain current RTD schedules Coordination Responsibilities: • Maintain thorough knowledge of the social service network in Boulder County • Develop working relationships with case managers from other agencies • Communicate via Staff Log, update client information in various data bases as well as physical files. Maintain files on resources and update contact information • Attend all staff meetings, clinical meetings, supervision, and process groups Education & Experience: • B.A. in Social Work, Psychology, or related field • Clinical experience with homelessness and related issues including substance abuse, mental illness, criminal justice involvement, and poverty • Spanish fluency preferred Skills & Abilities: • Ability to be flexible and adaptable • Ability to manage multiple projects and priorities • Demonstrated ability to work independently, be self-motivated, maintain strong boundaries and practice excellent self-care • Excellent one-on-one client skills Behaviors: • Commitment to maintaining confidentiality • Compassionate, people-oriented, with a strong sense of relationship building • Represents Boulder Shelter for the Homeless in alignment with the mission and core values: treat all with dignity, offer support and opportunity, achieve results through competence and creativity, and practice responsible stewardship Must be able to remain in a stationary position for long periods of time, exchange information, and move about work spaces.
