EmergeOrtho Triad Region welcomes all student-athletes to attend who are required to complete a NCHSAA PPE form for athletic participation. Who Should Attend: Any student-athlete who is required to complete a North Carolina High School Athletic Association Preparticipation Physical Evaluation form for athletic participation. Attendees will receive a comprehensive evaluation to meet the requirements of the NCHSAA. Ticket Information: Early registration tickets are available. Be sure to Register Early as space is limited. Early registration tickets are $10.00 and same day registration tickets are $15.00. Early registration closes on Sunday, May 22, 2022. Required NCHSAA Document Information: Be sure to complete your NCHSAA Sport Physical Form. Please bring your completed form with you to the clinic!
