University Hospitals Sports Medicine experts are committed to providing the same level of care -- on and off the field -- to our scholastic University Hospitals Sports Medicine experts are committed to providing the same level of care -- on and off the field -- to our scholastic athletes as we do our professional athletes. These free sports physicals provide athletes with a multifacieted exam that includes vision, musculoskeletal and cardiac risk assessment screenings. If any further information is needed for clearance, an appointment can be scheduled on-site. Please remember to bring your insurance card. This exam enables the athlete to participate in scholastic sports as required by the Ohio High School Athletic Association. It does not replace the annual well-check-up with their pediatrician. If you are unable to attend the free sports physicals event, please call 1-866-UH4-CARE for an appointment with a UH Rainbow Babies & Children’s primary care physician or nurse practitioner near you Please print, complete and have a parent or guardian sign the physical evaluation form; parents/guardians may also bring the form with them to the physical. You must also bring any relevant medical records.
