Join fellow medical professionals for a dinner and educational opportunity to explore partnerships between providers and pharmacists. We invite you to register for our no-cost educational event - More Time to Care: Leveraging Pharmacists for Better Patient Outcomes - on June 28th, 2022 at 4:00 PM. Hosted by NH DHHS, Division of Public Health Services, Chronic Disease Section, this event will be held at the Grappone Center in Concord, NH. Dinner will be provided. This is a great opportunity for physicians, nurse practitioners, physician assistants, practice managers, pharmacists, and others from ambulatory care settings who are interested in exploring how to improve patient outcomes by expanding partnerships and building more efficiencies into their practice. There will also be a discussion related to policy-based considerations and resource needs for implementing provider-pharmacist partnerships, including collaborative practice agreements. Why providers and pharmacists should attend:Hear from local and national medical experts who are successfully championing innovative team-based approaches to improve patient careDiscover proven strategies for the delivery of care in specific cases that shift tasks such as medication instruction and support to the pharmacistConnect with other providers who are interested in collaborating with pharmacistsEstablish communication mechanisms between providers and pharmacists to support care coordinationReduce burnout Continuing education credits:The planning committee will apply for CMEs & CPEs.
