Join us as we celebrate two Aetna Community Heart Award Winners- Sacramento Covered and Sacramento Self-Help Housing Providing access to services for the unhoused helps improve our community, and these award winners lead by example. During this luncheon, awardees and thought leaders will discuss new homeless services provided through Medi-Cal, as the State of California seeks to have Medi-Cal Managed Care Health Plans partner with local organizations for Enhanced Care Management and Community Support Services for the unhoused. The State's new initiative, Advancing and Innovating Medi-Cal (CalAIM), seeks to offer more social services as healthcare services, and address Social Determinants of Health like housing. Aetna looks forward to engaging with stakeholders to celebrate the early wins of this program and identify gaps to improve in the future. *Note, COVID-19 vaccination required for all attendees.
