Need a 5' x 12' Concrete Sidewalk Built

I need someone to construct a 5' x 12' concrete sidewalk in my front yard. The sod has already been removed from the area and the layout and most of the required excavation done. This needs to be a very professional looking job as our Home Owner Association is very strict. If you have the skills and experience to do the job and are interested please text me along with photos of your completed projects. If you can do stamped decorative concrete that is a plus. I have attached a couple of photos where the sidewalk is going.
