(Copy and Paste if needed) This survey is being conducted by the New York Academy of Medicine as part of a community health needs assessment for NewYork-Presbyterian (NYP), a network of hospitals and providers across New York City and Westchester. The purpose of this survey is to identify health issues that are important in your community. The information that you provide will help NYP to develop health services and programs.
