From the Town of Sudbury: September 8, 2020 Sudbury residents Only, drive thru flu clinics. Please select a Flu Clinic date, click on the desired clinic link, follow the instructions and sign up. Clinic 1. Seniors Citizens Only 65+ Wed. Oct. 7th 1:00 pm-4:00 pm Clinic 2. Town Wide Sat. Oct. 17th 10:00 am-4:00 pm Clinic 3. Town Wide Wed. Oct 21st 1:00 pm – 4:00 pm Clinic 4. Tentative (Dependent on Supply of Vaccine) Wed. Oct. 28th 1:00 pm – 3:00 pm Currently there is no sign up for this clinic. Check back in mid-October. You Must Bring a photo copy of your insurance card and a completed insurance form. Below are Adult and Children's Insurance Forms. Please click on the link below to download the appropriate form. One form is required per person: Please read all information carefully prior to clinic. Included is the required vaccine information statement and drive thru clinic check list.
