LPN for One on One Care for young individual Licensed Practical Nurse for Vaccination Administration ESSENTIAL FUNCTIONS: Reviews Medical Record for vaccination administration after receiving order from provider. Addresses any concerns with ordering providerObtains correct vaccination(s) from refrigerator. Compares and confirms vaccination(s) and dose ordered to what was retrievedCorrectly identifies patient in exam room using name and date of birthAdministers vaccine(s) as orderedDisposes of medical waste and hazardous waste, as per OSHA guidelines and clinic protocolsDocuments vaccine type, manufacturer, lot number, and anatomical location of administration in the EHRProvides patient and/or guardian with most current Vaccination Information Sheet (VIS)Updates new VIS when notified by CDC and shares with Satellite site MAsReviews CIR, generates list of needed vaccines and provides completed list to Front Desk/Registration Staff for recalls.Updates and revises CIR of any patients’ information and scans vaccine records into eCWMonitors and records twice daily refrigerator and freezer temperatures, as well as minimum and maximum refrigerator and freezer temperatures, resetting the memory each timeAssists MAs at Satellite sites with any questions/concerns as well as performs satellite-site visits for vaccine/refrigerator compliance when assigned.Prepares Excursion Incident reports in conjunction with all Shelter site MA’s or other LPNs; calling manufacturers and sending completed reports to Director HIM & QI for review before she shares with DOH.Prepares a Clinical Inventory Sheet (CIS) of the monthly inventory of all vaccinations at assigned siteAt LIC site, stock medication cupboard will be checked monthly for any expired medications; rotating oldest medications to front; discarding those already expiredReports VFC expired or spoiled vaccines electronically in the CIR for each specific site, and coordinates packaging of spoiled vaccines for pickup by the delivering entityEnters newly received vaccine information into eCW; i.e. vaccine name, manufacturer, lot number, expiration date, quantity.Follows and demonstrates strict infection control guidelines to ensure proper clinic environmentFollows sharps policy and procedure and demonstrates understandingParticipates with clinical staff for in-service training, to ensure that nursing staff functions meet quality improvement standards.Implements recommendations from Quality Improvement Team and Infection Control Committee, as requested
