Uber or Lyft driver needed for specific dates.

I need an Uber or Lyft driver to take me to and pick me up from cataract outpatient surgeries. Very minor procedure, but has been lawyered up so that the ride provider must be known with name and phone # provided ahead of time. No, you don't have to provide any care whatsoever; just pick me up on time in the morning and very important to pick me up a couple of hours later when you get the call. On the return trip you'll be given a rough estimate then get a call with about an hour's notice so you don't have to hang out in the parking lot. We will chat ahead of time so we actually 'know' each other. I will need 3 appointments / 6 rides total. One round trip on May 16th, May 20th and June 6th. It's very important that you show up when agreed. For your reliability and help I will show gratitude $ beyond the normal fare.
