Concrete Driveway Approach tear out and replace.

I am looking for a concrete specialist that is bonded to tear out and replace a 21' wide X 17' long section of my driveway. The area is the approach or the area that touches the street so I would need someone that is bonded in order to get a permit from the public works to do the work. I also need my driveway widened about 13' X 40' on both sides and a 20' long (curved) X 4' wide sidewalk torn out and replaced. (SEE PICTURES) I have had a couple of quotes but not from someone who is bonded and can take care of the permit. I am looking to have this done as soon as possible, weather permitting.
