Level concrete slab floors, install vinyl plank

I have about 1700 sq foot of floors on a slab. The slab is very unlevel. It needs to be levelled. Then, I am putting in vinyl plank flooring. It already has underlayment on it. I am looking for someone who is great at levelling floors and great at installing. Optional, you can also bid on doing crown and base. But right now, I want the floor install.
