Setup your own work schedule. Telephone or GPS Clock-In System. ​ Receive up-to-date status of Care Hours when you clock in. If they do not have Medicaid, apply online, we can help. Have Medicaid but not Homecare, submit a Home and Community Based Referral Form. The State will call him/her to do a screening to set up a Care Plan if they are eligible. If they already have a Homecare Care Plan, submit a PCCP Request Form to switch Providers. The State will switch them to 1st Priority Homecare.
