The Community Transformers Foundation (CTF) invites you to participate in this elegant and meaningful awards ceremony on Friday, June 3, Last year, our fundraisers enabled programs such as COVIDconnect to administer over 400 COVID-19 vaccines in over 17 zip codes. In addition, it also helped the TransConnect program to drive over 25,000 miles to distribute 9,765 meals to community members located in Dinwiddie, Sussex, Petersburg, Hopewell and Richmond. The Community Transformers Foundation was established to advance health and social equity. The Foundation seeks to address the social determinants of health, promote wellness and equality in communities that are historically underserved and underrepresented. Community Transformers Foundation is a private independent 501 (c) (3) organization serving communities in the Commonwealth of Virginia.
