Bathroom Remodel Needed

Hi I have a bathroom about 5 feet x 3 feet that I want to remodel. I want to put in a new floor, new sink/ vanity, toilet, mirror, light, and fan. I am not interested in changing the shower area (not included in the 5x3 size). Can you send me the following? 1) Your name and contact information? 2) Where are you located? 3) Are you licensed (I am opened to working with someone with or without a license)? 4) What do you charge to replace a toilet (including pick up, removal, materials, installation)?
