Need sidewalk lifted or replaced

Sidewalk has sunk over the years and when it rains due to landscaping from neighbors water kind pools on the sidewalk near the steps to the home. Open to ideas to correct (see pictures) Please share with me your idea with a rough estimate of how much to fix. The length is 16 feet x 28 inches wide.
