By establishing an advance directive, you can rest assured that you'll receive care on your terms because all the decisions made will be your own. You are invited to National Health Care Decisions Day sponsored by Sharp HealthCare in partnership with our District. The theme for this year’s event is Planting Now for the Journey Ahead. You will learn and be given the tools to: • Carefully select a substitute medical decision maker should you ever become unable to make your own health care choices. • Understand and complete your advance health care directive. • Ensure that your wishes for care and treatment are understood. Also, there will be an opportunity to start filling out an advance health care directive, as well as familiarize yourself with other available resources.
