A gathering to celebrate our accomplishments and look forward to sustaining our work in improving social determinants of health. Join us for the Mesa County Accountable Health Communities Model (AHCM) Consortium! This year, we will celebrate 5 years of implementing the Accountable Health Communities Model and learn from lessons along the way. This event will be an opportunity to connect with other practitioners, learn about the latest in AHCM and Rocky Mountain Health Plans, and develop tools to address the developing needs of our communities.
